The Health Priorities

During its third year, the Primary Care Trust (PCT) has made excellent progress in many areas of service.

After looking closely at the health needs of the local population, the Trust identified three main areas for health improvement. These were Coronary Heart Disease (CHD), Teenage Pregnancy and Diet and Obesity. Whilst significant improvement in these areas can only be achieved over the long term, some significant achievements have already been made. You can read about what we have achieved together with the views of Service Users on Page 36.

We can report many other exciting developments, which are making a real difference to patients from appointing specialist diabetes nurses to opening up new NHS dental centres and projects to keep our community hospitals safe and clean.

The PCT has also been working hard with local partners, patients and members of the public to expand cancer screening, develop outreach services for people with severe mental illness and expand rehabilitation services for older people when they leave hospital.

To help us better understand the priorities of our local communities and to respond to their health needs, we created a 'patch' based management structure led by Primary Care Development Managers. Eight Patch Teams have now been formed across the area, involving front-line health and social services staff and local people in decision-making about the health needs of their communities.
